Basic Direct Primary Care
Kids 0-18: $60 per month
Kids 0-18 with a paid adult: $40 per month
Adults 19-64: $80 per month
Family with 4 members: $200 per month
Add $20 for each additional family member
*One time enrollment fee: $100 adults, $50 kids
This is primary care for patients with no acute symptoms/ concerns, no deep dive needed to figure out problems/ symptoms. This is ongoing care with a focus on disease prevention and wellness. Great for healthy kids or adults just wanting the benefits of DPC. Unlimited visits and full access to providers via email, text, and phone. Includes wellness exam, screenings, same day or next day sick visits, prescribing, medication management, and BHRT. If chronic unexplained symptoms are present you will need the following option.
Basic Direct Primary Care + Chronic Symptoms
$300 initial visit (month 1)
$150 per months 2-5, then price decreases to DPC billing above
This is primary care with an intense 6 month deep dive into chronic symptoms and unexplained issues. Membership is higher in the first 6 months, then decreases to basic direct primary care membership cost after 6 months (sooner if symptoms resolve). Unlimited visits and full access to providers. Includes everything in option one.

Additional Information
We accept patients without insurance or with private insurance, but we DO NOT bill insurance (except for basic labs through LabCorp). Your membership fees cover the costs of all your visits. Insurance may be used for any outside labs, imaging, specialty referrals, or hospitalizations.
Not accepting patients with Medicare/ Medicaid at this time.
No long term contract required. All memberships may be cancelled at anytime with a 30 day notice.
No vaccines in stock at this time. Vaccines will need to be obtained through a pharmacy or the health department.
